“Hi, guys! Dr. Yalamanchili here. Today, I have a rhinoplasty patient, a 37-year-old female who’s bothered by the dorsum of her nose as well as its rotation. We’re going to bring down her nasal dorsum and rotate her tip as well. Let’s head over to the OR and get the day started.

Here we are, about to take down her cartilaginous dorsum. The hump of a nose has a bony part and a cartilaginous part, and here, I’m using a knife to take down the cartilaginous part to improve her overall contour. This is the cartilage septal extension graft—a piece of cartilage from her septum that I’m placing at the tip. This will give her support to rotate her tip. Here, we have her tip cartilages sutured to the graft.

At the end of the case, I’m molding her splint, which will stay on for six days. The packing inside her nose will be removed before she wakes up. We just finished in the operating room, and everything went great. She’ll recover for a couple of hours and then come back in six days to get her sutures and splint removed.
