People ask me about laser and IPL all the time. Having never received either of these types of treatment, I honestly had no idea what to tell them, except a half-hearted shill for the Mystical and Wonderful Powers of Google. Enter Sara Maxsell, a friend of one of our great clients. Sara works as an aesthetician at Bruno/Brown Plastic Surgery and was kind enough to answer some questions for us!

What’s the difference between laser resurfacing treatments and IPL? A lot of people (including me!) get confused around this terminology.

Confusing the two treatments is very common, as many times, they are just different hand pieces on the same machine. IPL stands for Intense Pulsed Light, and is NOT a laser. IPL shares some similarities with laser treatments, however, in that they both use light to heat and destroy their targets. IPL is used for hair removal, pigmentation issues, acne, and rosacea (to name a few). As for Laser Skin Resurfacing, there are several different types; here in our office, we use a fractional non-ablative laser, called The ResurFX. “Non-ablative” means that unlike other lasers, ours does not remove any skin; the laser penetrates to the deeper layers of skin to stimulate collagen growth without damaging the skin’s surface.

You assist with and consult on laser appointments. What does a consultation for that look like?

Generally, patients with skin concerns will start by coming to me for a complimentary consultation. Many patients are unsure what they want/need, and are looking for some guidance. During the consultation, I determine lifestyle habits, current skincare regimen, schedule, concerns, etc., and I can make my recommendation from there. On occasion, the patient’s concerns can be as simple as mild redness or a facial vein that bothers them (easily corrected with a few IPL sessions), but more often than not, they are also struggling with a whole slew of issues — anything from collagen loss, to severe wrinkling, to melasma, and more, all of which can require a more aggressive approach.

We are currently offering a comprehensive evaluation with Dr. Bruno or Dr. Brown and me. We are able to take the time to discuss a wide variety of issues and concerns, and answer any questions a patient may have. During this evaluation, we come up with a treatment plan and work together to accomplish their goals for the skin, and this plan usually involves a blend of several different treatments over several different sessions to achieve the best result. This comprehensive evaluation is something we are really excited about because we are able to take aging head-on, and correct and prevent in every way we can. There is nothing better than seeing someone confident in their own skin again, and we really pride ourselves on improving not only the aesthetic of the skin, but the overall health.

What’s your number one, most frequently asked question about IPL treatment? (And what’s the answer?)

“Why can’t I just get one treatment?” The answer is that pigmentation issues are not just on the surface. There are so many layers to the skin, and a lot of damage is underlying. IPL isn’t just a quick cure for cosmetic issues; we are also removing a lot of underlying damage to prevent it from popping up in the near future. Accumulative sun damage isn’t just black and white; the pigment can sit in different layers and some can be quite stubborn, which is why we recommend doing six sessions, to ensure efficacy and long-term success.

What’s something that no one asks, but should know before investing in a course of IPL?

Not many people ask what they can do to prevent future damage. I always recommend being on a pharmaceutical-grade tyrosinase inhibitor, generally a stable ascorbic acid (Vitamin C). This prevents the formation of brown spots, and ascorbic acid is a potent antioxidant, so you’re also getting protection against environmental aggressors. The most important thing to always do is wear a broad spectrum SPF. I’m sure people get tired of me saying that, but I always make sure to add that in!

And now something no one asks (but should) about laser resurfacing treatments?

Not many people ask what they can do to continue results. Now, with the laser, you are stimulating collagen and elastin on a dermal level to repair and renew the cells and bring new fresh skin to the surface. Chemical exfoliation is a great step you can do to maintain results as the acids penetrate to loosen the “intercellular cement,” allowing new cells to turn over at a rapid rate and bring new skin to the surface. Stimulation to those deeper layers is key, so I always suggest my patients come in for a peel every few months, use a prescription-strength Vitamin A derivative, or use a daily regimen of a pharmaceutical-strength exfoliant to have that constant stimulation.

What’s the range of fees someone might encounter when it comes to IPL?

For one session on the face and neck, IPL is $350, but we do suggest purchasing a package of six sessions for $1,400 to maximize results and minimize future pigmentation issues. We also offer a photo-fraction, which is a combination of IPL and Laser Skin Resurfacing in one session. A one-time laser treatment is $800, but we do suggest a package of four for $2,600, to ensure you receive the very best results, and set your skin up for future success.

Patient Newsletter Addressing the Link Between Breast Implants and A Rare Form of Cancer

Breast augmentation and reconstruction are two of the most popular plastic procedures among women. With growing concerns about a link between breast implants a rare form of cancer, Bruno|Brown Plastic Surgery released this patient newsletter:

As a patient in the practice of Bruno|Brown Plastic Surgery, we feel it necessary to inform you of recent issues with breast implants. Many of you have now seen or read recent media reports referencing the possible link between breast implants and a rare form of cancer called BIA-ALCL (Breast Implant Associated-Anaplastic Large Cell Lymphoma). While this is occurring in an extremely small subset of patients with breast implants, there appears to be some link.

BIA-ALCL is not a breast cancer, but a rare T-cell lymphoma that usually develops as a fluid and swelling around the textured shell breast implant. According to the ASPS and the FDA, thus far, there have been no confirmed cases of BIA-ALCL in women who have had only smooth-shell breast implants. The lifetime risk for this disease appears to be about 1 case for every 30,000 textured implants placed. This equates to a 0.003 percent risk. Both the FDA (Food and Drug Administration) and ASPS (American Society of Plastic Surgeons) and others are gathering more and more data on this rare disease.

The FDA is not recommending removal of textured implants nor have they advised against their placement. Rather, the FDA recommends, as do we, that every woman conducts regular self-examinations. If you develop swelling or a lump in your breast, contact our office right away. We will comprehensively evaluate you and order the appropriate tests to determine if any treatment is indicated.

While more information is being gathered, we at Bruno|Brown Plastic surgery have elected to stop the use of textured implants from all breast manufacturers until further research on this type of implant is more complete. SculpSure vs CoolSculpting: Which Non-Surgical Body Sculpting Procedure Is Right For Me?

With so many procedures, how can we choose? Dr. Bruno answers our most common questions SculpSure.

 

Q: Does it hurt during the SculpSure treatment?

A:

There may be a mild sensation of the area being treated, but there is no pain. Patients are fully awake for the treatment, and it takes less than 30 minutes to have an area treated.

Q: Are the results permanent?
A:

Yes, results are permanent as long as one maintains a reasonable range of body weight, healthy living with good dietary selections and an active lifestyle.  

Q: How much weight can I expect to lose?
A:

According to the official SculpSure website, each 25-minute procedure can destroy up to 24% of targetted fat cells. It’s important to note that SculpSure, along with any other fat removal treatments (liposuction, Smartlipo, Coolsculpting), are not meant for weight loss but rather shaping and refining problem areas where fat deposits. Combined with a healthy lifestyle, good dietary selections, and an active lifestyle, one would expect some weight reduction. 

Q: What areas can I target with SculpSure?
A:

Many areas are amenable to treatment.  The most common are the abdomen, hips, flanks and thighs.  

Q: Are there side effects for either CoolSculpt or SculpSure?
A:

Both treatments are non-invasive methods of fat reduction.  It is not uncommon for patients to have bruising and itchiness after Coolsculpting. There may be some hypersensitivity of the area treated for a short time after the SculpSure, but patients will be able to go about their daily routine.

Q: What are the primary differences between SculpSure and CoolSculpt treatments?
A:

SculpSure uses topical laser treatment to reduce the underlying fat whereas Coolsculpting freezes the fat.  The treatment time is much shorter for Sculpture, only 25 minutes. CoolSculpting takes an hour per treatment. Another difference between the two procedures is the possible side effects. After SculpSure, patients may experience a slight tenderness. After, CoolSculpt patients have reported tenderness, slight bruising and itchiness.

The Impact of Animation Deformity on Quality of Life in Post-Mastectomy Reconstruction Patients

Background: Animation deformity after subpectoral breast implant placement has been documented; however, the actual prevalence and effect on patient quality of life has not been studied much.

Objectives: The purpose of this study was to show that all patients with subpectoral implants experience some degree of animation deformity and that it can affect their quality of life, including causing embarrassment and discomfort in reconstructed patients.

Methods: Patients who underwent breast implant surgery were contacted for inclusion in the study. Patients were obtained from a single surgeon’s practice but included patients operated on outside the practice, and those seen in the practice for a consultation. A six-item questionnaire was developed by the senior author (H.B.) and the medical student (N.F.) involved in the study, to assess quality of life related to animation deformity. Patients had their degree of animation deformity assessed by the senior author and a medical student.

Results: Of 25 patients who agreed to the questionnaire and assessment, 20% had grade I distortion, 44% grade II, 24% grade III, and 12% grade IV. Of the patients questioned, 80% were bothered by an animation deformity and 45% of those patients were bothered to a significant degree (≥6 out of 10). In addition, 48% of patients felt that the animation deformity interfered with their daily life, and 28% (7/25) of patients underwent, or were scheduled to undergo, revision of their reconstruction at the time of interview. The degree of the clinically observed animation deformity was correlated with patient dissatisfaction, with an R value of 0.47 (P value = 0.0145).

Conclusions: All patients with subpectoral implant positioning will experience some degree of animation deformity. Especially in the reconstructed breast population, animation deformity, and its severity, affects patients’ quality of life. Other approaches to reconstruction should be considered to prevent animation deformity in this population.

Botox-resultsThere’s a lot of misinformation out there about when to use Botox. We’ve heard and seen just about everything. Here, Dr. Brown sits down to answer the most common questions we get about Botox.

Q: When is the best age/time/signals you need to start considering Botox?
A:

As one might expect, people are very expressive, even in their sleep or just talking normally. The best time to start isn’t a specific age, but is when you notice that your expressiveness is leaving small creases in the skin.

It is easier to prevent than to correct, so patients are coming in earlier for preventative maintenance that keeps fine lines and wrinkles at bay for much longer. Coupled with good skin care and protection from environmental damage, the aged appearances of the past are becoming less frequent.

In our practice, most patients are in their 30s to 60s, with the bulk in the 40-55 range; I think it’s trending lower as more people realize it’s easier to prevent than to correct, so patients in their late 20s and early 30s are coming in, but maybe every 6 months instead of every 3-4.

Q: How should a patient consider using Botox in their 20s/30s/40s and so on?
A:

• 20s: It’s more preventative, so consider having it every 4-6 months
• 30s: You’re starting to correct wrinkles that show without moving, and these patients typically need it every 4 months
• 40s: This is more of a combination age, and these patients may need additional correction in the realm of skin care/PRP/etc; maybe having their eyes done and so forth to really help freshen the appearance
• 50s and up: This age starts to err more in the direction of correction (surgery) and Botox is more “icing on the cake” so to speak.

Q: Does it just work on the forehead and crow’s feet?  Or are there other uses to consider?
A:

For me, the vast majority get it between the eyes and on the crow’s feet; next is the forehead and then ancillary areas such as the corners of the mouth, the bunny lines on sides of nose, neck bands and more. If you’re concerned about a particular set of wrinkles you’ve noticed and wonder if Botox is the right treatment, you should set up a consultation to discuss your specific needs.

Q: What gives some people the droopy eye and /or frozen look?  How do you make sure you avoid having that happen?
A:

The tendency to treat fully the forehead lines allows the brow to drop, thereby giving the eyes a tired/droopy and frozen look. I go lighter on the forehead for this very reason; balance it with the elevation from the frown lines and the crow’s feet and it looks more natural.

Although we aim to get a perfect result every time, it’s often better to under correct as you can always add more if needed.

Q: At other med spas we’ve seen it as common practice to do big shots that leave us with a headache and the ‘mosquito bite’ telltale sign.  Can you explain your approach?
A:

Every doctor has their own way but we use one of the smallest needles available and perform more precise injections in the muscle versus larger amounts; while there is always a chance to have a bruise or achy spot, using these techniques coupled with the application of ice minimizes our patients discomfort.

Q: How long do the results last?
A:

Results last about 4-6 months in certain areas, some people burn it off quicker; most patients return every 3-4 months.
